Adolescent Classes Abstinence Classes - The purpose of this class is to teach youth the value of abstinence. Male Outreach - The purpose of this class is to reach out to males with the abstinence message. Puberty Class - The goal of this class is to educate, support and empower families of pre-teens. The parents will learn about communicating with their children, puberty, and the dangers of early sexual activity. The children will learn about the changes they should expect as they grow and ways to handles those changes. Diabetes Classes Diabetes Basics - Recommended for diabetics, their families, and those at risk. Topics include review of diagnosis, how to minimize or avoid potential for long term complications, setting blood pressure and cholesterol goals, managing high and low blood sugar, and measures to promote a healthy lifestyle. Diabetes Meter Assist - Mandatory class needed to obtain new glucometer (blood sugar monitor)-Precision Xtra, through the Cook County Neighborhood referral program. Classes supply patients with meter and 10-50 testing blood sugar strips upon completion. Includes orientation to proper use, normal blood sugar values, and correct documentation for provider. Individual counseling available on a limited basis per request. Patient will receive proof of attendance and in obtaining new meter, which should be written on their next prescription for CCH personnel. Class by referral only. Diabetes Nutrition - Teaches food groups, basic carbohydrate counting, and emphasizes concepts of “mixed meals,” meal “spread,” and appropriate portion sizes. Geared for patients with diabetes or pre-diabetes/glucose intolerance. Fast paced class, not for young children. Diabetes Support Group - Discussion group for those affected by diabetes. Fats, Fiber, Exercise Classes Covers fats and fiber in detail. Emphasizes exercise and portion control with good nutrition. A must for patients with diabetes, high blood pressure, cholesterol or cardiovascular concerns. Highly recommended for patients desiring to control or lose weight. Parents to attend in place of child less than 12 years. Fast paced learning environment not for young children. This class completes the basic dietary teaching for diabetes. High Blood Pressure Classes Recommended for patients with high blood pressure and those at risk. Education about having high blood pressure, blood pressure values, weight management, medications, and ways to effectively manage high blood pressure to avoid long-terms complications (including foods to emplhasize and guidelines for reducing salt and sodium consumption). Materials individualized to patient self-management needs. The Fats, Fiber & Exercise Class is highly recommended in addition to complete one's education on good management. Maternal and Child Health Classes Childbirthing Classes - Discuss topics like nutrition, dangers signs of pregnancy, labor & delivery, and breast feeding (Classes are offered in both English and Spanish). EMDTY (Embracing Motherhood During Teenage Years) Support Group - Supportive, educational group for pregnant teenagers, ages 13-19. Prenatal Support Group - Supportive, educational group for pregnant women ages 19 and above. Parenting Classes/Discussions Fathers Support Group - Discussion group for fathers, or soon-to-be fathers. Infant and Toddler First Aid - Infant and toddler first aid focusing on accidents. Discussion Groups - For parents with children age 3 and under. Topics discussed: what happens at a well child visit, developmental milestones, safety, nutrition, potty training and discipline. (English only) Positive Discipline, Your 0-3 Year Old - A discussion group for parents with children age 0-3. Discussion will take place around positive discipline strategies. Protecting Your Child from Physical and Sexual Abuse - A discussion group for parents with children age 0-3 about preventing child abuse. Potty Training Your Child - A discussion group for parents with children age 0-3 about potty training. Smoking Cessation Classes Eight-week program for adults certified by the American Lung Association. Participants must have motivation to quit and assessment for readiness will be conducted. Class topics will include benefits of smoking cessation, creative ways to combat cravings, stress reduction methods, and relapse prevention ideas. Certification available upon completion of class and group and/or 1:1 counseling also available. Support Groups HIV Support Group - This group is an open support group for HIV-positive individuals. While occasionally a specific topic or educational session will be held during the meeting, the group is generally discussion-based, providing participants a chance to share their struggles and experiences as people living with HIV. Men’s Substance Abuse Support Group - This group offers education and support for recovering substance-abusing men, focusing on topics including anger, addiction, and relationships. The group is offered in a seven-session format. Interested participants must meet with the group facilitator individually before attending their first group session. Women’s Sobriety Support Group - Ongoing fellowship and support for women in recovery from alcohol and drug addiction. This group offers women a safe place to form positive relationships with other women and be renewed and encouraged in their commitment to remain sober and drug-free. |
