Supervisor: Nicole Milano Leyva, M.S.W.
Nicole obtained a Masters Degree in Social Work from the University of Illinois at Chicago, with a concentration in Community, Administration and Policy. Nicole received her clinical license in 2005. She is also a certified lead risk assessor. Nicole has been at LCHC since October 2001 where she provides leadership to MCH Pediatric Team. Health Support Worker II: Tasmania Pegues
Tas obtained her Bachelors Degree in Human Services from National Louis University. Tas has over 10 years of social service experience working with children and families. Health Support Worker II: Erminia Garcia

Health Support Worker I: Candice Dowden-Muhammad
Candice received her bachelors degree from National Louis University, majoring in Applied Behavioral Science. Health Support Worker I: Jesus Rodriguez

Lead Educator: Carlos Arrizon
Carlos obtained his bachelors degree from Louisiana Tech University with a major in sociology. He is a certified lead risk assessor. Lead Educator: Mavis Woodard
Mavis has been a Licensed Lead Risk Assessor since 1993. She has worked in the City of Chicago with lead poisoning issues for over 10 years! Mavis has been at LCHC since March 2003. M.S.W. Student Intern – Elisabeth Gainer
Libby received her bachelors degree from Miami University in Ohio, majoring in Sociology. She is at Loyola University Chicago studying for a Masters Degree in Social Work. She completes developmental screenings with children ages 4 months to 5 years old and is also working to implement LCHC’s new pediatric obesity program. M.S.W. Student Intern – Veronica Carlson
Veronica received her bachelors degree from Southern Illinois University, majoring in Criminal Justice. She is at University of Illinois – Jane Addams College of Social Work studying for a Master’s Degree in Social Work. She completes developmental screenings with children ages 4 months to 5 years old and also provides case management to a small case load of pregnant women.
