Service Line Provider: Dr. David Miller
Dr. Miller completed medical school at the Ross University in 1988. He is currently licensed as an allopathic physician in the state of Illinois, specializing in family medicine. He has worked at Lawndale Christian Health Center since 2000.

Supervisor: Allison Pizzi
Allison received a bachelors degree in Social Work from Taylor University in 2002 and a masters degree in Social Work from Case Western Reserve University in 2003 with concentrations in social policy and substance abuse. Health Support Worker – Substance Abuse: Linda Johnson
Linda has been working in Chicago in the substance abuse field her entire career. With a solid understanding of substance abuse and addiction coupled with a heartfelt sense of compassion for the people she serves, Linda has been with LCHC since 2000.
